Medica is seeking a Registered Nurse Case Manager to oversee North Dakota Medicare Advantage DSNP members in a remote role. You will educate members, coordinate care across the care team, and support social determinants of health through community resources and referrals.

Required are an active RN license, 5+ years in acute/home/community care, and experience with EHR systems. This role offers a broad rewards package and state eligibility considerations.

Qualifications

• RN license in state of residence (unrestricted)
• 5+ years of experience in acute, home, or community-based care
• Experience with EHR/clinical documentation platforms
• Experience with diverse populations and settings (elderly, disabled, varied backgrounds)

Responsabilités

• Educate members and caregivers about care plans and CMS requirements
• Manage medical case management interventions for DSNP members
• Collaborate with member's care team and community resources
• Support social determinants of health through resource identification and referrals
• Perform other duties as assigned
