Qualifications

• LCSW or LSW licensure in Indiana required.
• Master's degree in Social Work required.
• 2+ years of inpatient, outpatient, or home-based social work experience required.

Responsabilités

• Assess, plan, implement, coordinate, monitor, and evaluate services across the continuum of care to ensure quality patient outcomes.
• Support smooth transitions between levels of care through discharge planning and care coordination.
• Collaborate with nursing staff and other departments to facilitate effective discharge planning and barrier resolution.
• Document patient records timely and accurately to ensure continuity of care and adherence to clinical pathways.
