Job summary

Fraser is seeking a dedicated Case Manager to serve individuals with developmental disabilities and mental health needs in Minnesota. The role focuses on client advocacy, connecting clients to community resources, and thorough documentation.

The position requires a bachelor’s degree in a related field and experience with DD populations or completion of Relias I/DD course. Travel to client sites is expected, with a focus on compassionate, inclusive care.

Qualifications

• Bachelor's Degree in social work, special education, psychology, nursing, human services or related fields.
• At least one year of experience with DD populations or willingness to take Relias I/DD course (paid by Fraser).
• Experience or interest in working with adults with serious mental illness.
• Commitment to diversity, multiculturalism, and inclusion with culturally responsive practice.

Responsibilities

• Provide client-centered case management services for CADI/BI and DD waivers.
• Advocate for Fraser clients and connect them to community resources.
• Travel to meet clients 2–3 times per week; caseloads around 35–43 clients.
• Document thoroughly and maintain detailed case notes.
