Oscar Health is hiring a Case Manager to join our Care Delivery - Clinical team. This remote role educates members, evaluates Social Determinants of Health, and connects them with community resources to improve health outcomes.

You will collaborate across Oscar teams, coordinate care across settings, and contribute to process improvements. Requirements include RN license or MSW and several years of care management experience.

Qualifications

• 2+ years clinical care management in hospital, outpatient, or community settings.
• 2+ years experience with patients with chronic conditions (diabetes, COPD, hypertension).
• 1+ years experience Care Coordination and Navigation.
• Active, unrestricted RN license or MSW from a CSWE-approved school.

Responsibilities

• Coordinate care across inpatient, outpatient, ER, home care.
• Partner with members and providers to navigate health challenges and create care plans.
• Assess SDOH by connecting members with community resources.
• Provide crisis intervention and advocate to resolve barriers to care.
• Engage with members via phone, secure messaging, or in-person to educate on health conditions, discharges, procedures, and prescriptions.
• Compliance with applicable laws and regulations.
• Other duties as assigned.
