Qualifications

• 2+ years of clinical care management experience.
• 2+ years working with patients with chronic conditions.
• 1+ years experience in care coordination and navigation.
• Active RN licensure in the US or a valid MSW degree.

Responsibilities

• Coordinate care across inpatient, outpatient, ER, and home care settings.
• Partner with members and providers to create personalized care plans.
• Assess Social Determinants of Health and connect resources.
• Provide crisis intervention and advocate for members’ access to care.
• Educate members by phone, secure messaging, or in person about conditions, discharges, procedures.
• Ensure compliance with applicable laws and regulations; perform other duties.
