CVS Health is seeking a Case Management Coordinator for a full-time telework role in Illinois with 50-75% travel across Northwest IL counties. You will assess member needs, coordinate care plans, and help members access benefits while collaborating with a team to improve health outcomes.

The role requires 2+ years in behavioral health or related fields, strong electronic documentation skills, and proficiency with Microsoft Office. Travel mileage is reimbursed; standard hours apply.

Qualificações

• 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsabilidades

• Evaluate members' needs and eligibility and recommend an approach to case resolution.
• Identify high risk factors and coordinate appropriate referrals to care management.
• Coordinate and monitor assigned care plan activities and progress.
• Collaborate with clinicians and managers to optimize quality of care and patient outcomes.
• Coach and support members to engage with providers and make informed health decisions.
