Qualifications

• 2+ years of case management experience with individuals with SMI and elderly.
• Ability to travel up to 50% within West Valley, AZ.
• Bilingual English/Spanish communication skills are preferred.

Responsibilities

• Coordinate, document, and communicate all aspects of the utilization/benefit management program.
• Develop and monitor care plans; identify high-risk factors and appropriate referrals.
• Provide coaching and information to empower members in healthcare decisions.
• Participate in case conferences and ensure compliance with regulatory guidelines.
