Job summary

The Kennedy--donovan-Center- is seeking a Shared Living Case Manager to coordinate individualized placements and monitor outcomes, ensuring health, safety, and personal goals of individuals served. Responsibilities include case management, skill training, supportive counseling, crisis intervention, home visits, and accurate documentation.

Qualifications

  • Minimum of 2 years’ experience in case management, direct support, or a related human services role.
  • Knowledge of person-centered planning, developmental disabilities, and community resources.
  • Strong organizational, documentation, and communication skills.
  • Ability to work independently while maintaining collaborative relationships with teams and caregivers, and community partners.
  • CPR/First Aid Certified Trainer (or ability to obtain certification within designated timeframe).
  • Valid driver’s license, reliable transportation, and willingness to travel extensively.

Responsibilities

  • Coordinate and monitor individualized placements, ensuring health and safety of individuals served.
  • Perform case management, skill training, supportive counseling, crisis intervention, and home visits.
  • Develop service plans, provide caregiver support, and maintain accurate documentation.
  • Collaborate with individuals, families, caregivers, healthcare providers, and community partners.
