Job summary

Boston Children's Hospital is seeking a Case Coordinator to manage patient care workflows and develop assessment-based plans for pediatric clients. The role involves acting as a liaison and advocate across health, social service, and government agencies, with emphasis on empowering families to access resources.

Remote position based in Massachusetts with focus on southern region pediatric patients; Spanish language skills are preferred to effectively meet family needs and enhance outreach

Qualifications

• A high school level of educational development and one to two years of previous experience.
• The analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.
• The ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
• Spanish bilingual preferred.
• This is a remote position.

Responsibilities

• Manage case flow and develop assessment, problem identification and treatment plans for concrete client needs.
• Serve as liaison, interpreter and advocate for clients with health, government and social service agencies.
• Promote, maintain and improve health of patients and their families; provide social support and informal counseling.
• Support outreach with home visits and health screenings; engage with families in their preferred language where possible.
• Collaborate with the Patient Centered Medical Home team to coordinate care and connect families to resources.
