Job summary

Southwest-Key-Programs seeks a Career Coach to assist TANF, SNAP, ABAWD, and NCP participants. You will verify eligibility, assess needs, and develop employment plans while providing ongoing case management and maintaining strict confidentiality.

You will coordinate childcare, transportation, and other supports, monitor progress, and document in TWIST/WIT/COLTS. Occasional travel and telework options are available as needed.

Qualifications

• Prefers a baccalaureate degree plus one year case management experience.
• 60 credit hours with 3 years of full-time qualifying experience acceptable.
• Bilingual in English and Spanish preferred.

Responsibilities

• Provide information and verify eligibility for TANF, SNAP, ABAWD, and NCP programs.
• Conduct assessments to determine participant needs.
• Develop and track employment plans and provide case management while maintaining confidentiality.
• Coordinate supportive services such as childcare, transportation and work-related support.
• Conduct follow-up to monitor activities and progress.
• Prepare, submit and monitor case management documentation in TWIST/WIT/COLTS in a timely manner.
• May facilitate group orientations.
• Travel to attend meetings, training and outreach; telework as needed.
