Job summary

Alliance Health is seeking a non-licensed healthcare professional to support timely transitions of care across plans and provider-led entities. The role works within a multidisciplinary care team to deliver warm handoffs, reporting, and follow-up coordination.

Qualifications

• Bachelor’s degree in Human Services and at least 2 years working directly with individuals with Behavioral Health and Physical Health conditions.
• Or Bachelor's degree in a non-human services field and four years of experience working with Behavioral Health and Physical Health conditions.
• Preferred: NACCM, NADD-Specialist, Health Education Specialist, and/or CBIS.

Responsibilities

• Coordinate with internal teams and external partners to ensure timely, informed, and seamless care transitions
• Manage inbound and outbound communications related to transition planning, data accuracy, and care coordination
• Proactively identify and resolve transition-related issues to minimize care gaps and enhance member outcomes
• Completion of Transition Summaries
• Completion of warm hand offs with identified entities
