Job summary

Alliance Health Plan seeks a Director of Care Transitions to lead the Facility-Based Care Management team. The role focuses on coordinating care for members with complex physical and behavioral health needs during transitions between facilities.

The position is primarily remote with occasional in-office meetings in North Carolina. Requires a Master's degree and active NC licenses, plus several years of supervisory experience in health-related programs.

Qualifications

• Required: Master's degree in one of the listed fields and five years of related experience.
• Active NC Clinical License (LCSW, LMFT, LCMHC, LPA, or RN) is required.
• Multiple degrees may be accepted with supervisory/administrative experience.

Responsibilities

• Lead the Facility-Based Care Management team to support members with complex needs.
• Oversee care transitions to ensure continuity and minimize gaps.
• Develop staff through coaching, training, and performance feedback.
• Collaborate with hospitals, CMA and AMH+ care management models.
• Ensure compliance with licensure, regulatory, and accreditation requirements.
