Job summary

SCAN Health Insurance seeks a Care Coordinator - SCAN Temp to contribute to the care and well-being of SCAN members, empowering them to age in place by collaboratively assessing, planning, coordinating, and monitoring their holistic needs. Focus on addressing members’ care coordination needs, processing referrals and services, and supporting navigation of the healthcare system.

You will perform assessments, act as triage for benefits, use motivational interviewing, and document care plans.

Qualifications

• High School Diploma required; some college preferred.
• 0-3 years in healthcare or customer service.
• Ability to use motivational interviewing to address member concerns.
• Strong written and verbal communication skills.
• Knowledge of CMS and DHCS regulations is preferred.

Responsibilities

• Perform assessments and screenings to determine interventions and referrals.
• Act as triage point for administration of benefits and to troubleshoot access issues.
• Use motivational interviewing to understand member goals and concerns.
• Coordinate care utilizing benefits and community resources; resolve access issues.
• Document assessments, care plans, and notes in multiple systems within timeframes.
• Collaborate with county offices and agencies as needed.
• Demonstrate professionalism and empathy in all interactions.
• Support SCAN’s vision and goals; perform other duties as assigned.
