Job summary

Mom's Meals | A Purfoods Company seeks a Care Navigator to conduct social care screenings, eligibility assessments, and referral coordination for Medicaid members through telephonic navigation. You will document activities, manage consent, and collaborate with community and healthcare partners to ensure person-centered, trauma-informed care.

This remote role may require occasional travel to Iowa or New York offices.

Qualifications

• Minimum of two (2) years’ experience in a healthcare, social service, or behavioral health setting.
• Ability to engage individuals with empathy and professionalism.
• Ability to maintain accurate documentation in regulatory and organizational requirements.
• Prior telephonic interactions with clients or patients.

Responsibilities

• Conduct health-related social needs screenings and eligibility assessments.
• Coordinate referrals and follow-up within referral platforms.
• Document activities and time for program billing and reporting.
• Maintain HIPAA and confidentiality compliance.
