Job summary

Mom's Meals, a PurFoods Company, is seeking a Care Navigator to perform telephonic social care screenings, eligibility assessments, and referral coordination for Medicaid members. This remote role may work CST/EST hours and may require occasional travel to IA/NY offices. Hourly wage is $23/hour.

Responsibilities include screening, eligibility verification, referrals, documentation, and coordination with care teams while ensuring HIPAA compliance and program guidelines are followed.

Qualifications

• Minimum of two years’ experience in a healthcare, social service, or behavioral health setting.
• Ability to engage individuals with empathy and professionalism.
• Accurate documentation in regulatory and organizational standards.
• Prior telephonic or in-person client interactions.
• Excellent written, verbal, and interpersonal communication.
• Strong organizational skills with multitasking ability.
• Proficiency with standard office tech platforms.

Responsibilities

• Conduct health-related social needs screenings per program requirements.
• Complete eligibility assessments and supporting documentation.
• Initiate, monitor, and close referrals in referral platforms.
• Document and record service activities and time spent.
• Follow up on referrals and address barriers.
• Educate clients about resources, benefits, and services.
• Coordinate with providers, MCOs, and community orgs.
• Escalate complex cases to supervision when needed.
• Maintain HIPAA and confidentiality compliance.
• Meet productivity, timeliness, and quality standards.
