Job summary

Mom's Meals is seeking a Care Navigator to conduct telephonic social care screenings, eligibility verifications, referrals, and Ongoing care navigation for Medicaid members and related populations. This remote role supports CST/EST hours with occasional travel to Iowa/New York offices.

The position emphasizes trauma-informed, culturally responsive care and requires strong documentation, communication, and organization for timely, compliant service delivery.

Qualifications

• Two years in healthcare, social service, or behavioral health with direct client contact.
• Ability to document accurately in compliance with regulations.
• Experience with in-person or telephonic client interactions.
• Strong written, verbal, and interpersonal communication skills.
• Able to work independently with minimal supervision.

Responsibilities

• Conduct standardized health-related social needs screenings.
• Complete eligibility assessments and documentation for social care services.
• Initiate, monitor, and close referrals in referral platforms.
• Document service activities and time spent per billing requirements.
• Follow up to confirm service connections and address barriers.
• Educate clients about community resources and benefits.
• Coordinate with healthcare providers, MCOs, and community orgs.
• Maintain HIPAA compliance and confidentiality.
• Meet productivity and quality standards.
