Job summary

Mom’s Meals is seeking a Care Navigator to support Medicaid members and eligible populations through NY-based hybrid work with regular local travel. The role focuses on identifying health-related social needs and connecting people to essential services.

The ideal candidate will deliver trauma-informed, culturally responsive navigation across community events, telephonic, and field settings, reporting to the Care Navigation Program Manager.

Qualifications

• Minimum 2 years of experience in healthcare, social services, behavioral health, community outreach, case management, or related field.
• Experience engaging diverse communities with empathy, professionalism, and cultural humility.
• Ability to build rapport quickly in community-based and telephonic settings.
• Experience conducting direct client interactions in-person, telephonically, or through outreach activities.
• Strong verbal, written, and interpersonal communication skills.
• Ability to document accurately and maintain detailed records per regulations.
• Valid driver’s license, reliable transportation, and ability to travel regularly.
• Proficiency with laptops, smartphones, and field-ready technology.

Responsibilities

• Conduct standardized health-related social needs screenings across settings (in-person, community events, telephonic).
• Complete eligibility assessments and documentation for social care services and programs.
• Provide on-site navigation support at events, health fairs, and enrollment activities.
• Build relationships with community organizations, healthcare providers, and local partners.
• Serve as embedded navigator within partner organizations for referrals and coordination.
• Manage referrals in designated referral management platforms and close loops.
• Document activities, encounters, and outcomes per billing and reporting requirements.
• Escalate complex cases and safety concerns to supervisory or clinical staff.
