Mom's Meals is seeking a Care Navigator to support Medicaid members and other eligible populations in New York through SCN, 1115 Waiver, ILOS, VAB, and related programs. This role helps individuals identify and address health-related social needs by connecting them with services and resources.

The position offers a hybrid NY-based schedule requiring regular local travel, with occasional travel within New York State.

Qualificações

• Ideal candidate is comfortable engaging individuals at community events.
• Ability to build relationships with partner organizations.
• Provide ongoing navigation support by phone.
