Job summary

MasterCare, Inc. is seeking a Care Navigator to deliver non-clinical care management under the MasterCare Plan in Placerville and surrounding areas. The role combines in-person and remote work to support patients’ physical, behavioral, social, environmental, and financial well-being.

You will build relationships with providers, conduct assessments, develop care plans, and coordinate smooth transfers, while prioritizing person-centered care and practical solutions for seniors and their families.

Qualifications

• Bachelor’s degree or equivalent experience in healthcare-related fields.
• Three+ years in marketing and/or social services in healthcare or senior services.
• Experience with both clinical and non-clinical services for elderly populations.
• Ability to perform physical tasks and travel within Placerville/El Dorado County.

Responsibilities

• Serve as primary contact with local medical and nonmedical providers.
• Develop referral relationships and provider outreach to achieve company goals.
• Develop and execute the Master Care Plan for ECM and CS patients.
• Conduct in-home or facility assessments as required.
• Coordinate patient transfers and ensure seamless communication.
• Deliver person-centered care management in a non-clinical setting.
