Job summary

Mindoula Health in Nevada is seeking a Care Manager for its Suicide Prevention Program. You will provide virtual support and community-based services to individuals and families affected by suicide ideation and attempts, coordinating care and navigating healthcare resources.

The role demands crisis intervention skills, empathy, and collaboration with providers. This full-time position offers WFH flexibility and community-based work as needed within the Nevada region.

Qualifications

• Required: Bachelor’s degree in psychology, social work, or related behavioral health field.
• Preferred: Master’s degree in a related field.
• At least 2 years of experience in a related role with at-risk youth/adults.
• Crisis intervention training or similar certification required.
• Knowledge of local resources, social determinants of health, mental health, and substance use.

Responsibilities

• Member Engagement: outreach, engagement, and regular check-ins with a cohort of members.
• Care Coordination: develop and coordinate community resources for medical, behavioral health, and social needs.
• Crisis Management: assess risk, perform crisis interventions, ensure safety with tailored plans.
• Service Plan Development: assess options and update service plans leveraging benefits and community resources.
• Education & Advocacy: educate about health, abuse prevention, medications, and available resources; advocate between members, families and providers.
• Appointment Scheduling: schedule appointments and follow-ups to ensure attendance.
• Documentation & Reporting: maintain accurate records and case notes.
