You will conduct assessments, develop and monitor care plans, and facilitate waiver enrollment while navigating regulatory timelines.

Qualifications

Licensed professionals must be active and unrestricted in state of practice.
In some states, a bachelor’s degree in a health care related field may be required.
Experience in care management, managed care or LTSS settings preferred.

Responsibilities

Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
Facilitates comprehensive waiver enrollment and disenrollment processes.
Develops and implements care plans with members and care network.
Performs ongoing monitoring of care plan effectiveness and documents outcomes.
Promotes integration of services to enhance continuity of care.
Assesses medical necessity and authorizes waiver services.
Evaluates benefits and funding sources for members.
Facilitates ICT meetings for service decisions.
Uses motivational interviewing to educate and motivate change.
Assesses barriers to care and addresses psycho/social, financial, medical obstacles.
Identifies critical incidents and develops prevention plans.
May provide consultation and resources to peers.
Travel of 25-40% local may be required.
