Job summary

Molina Healthcare is seeking a field-based care management professional to support LTSS activities across the greater Cook County area. This remote role includes travel to member homes and coordination with a multidisciplinary team to deliver quality, cost-effective care.

You will develop and monitor care plans, assess medical necessity, and collaborate with caregivers, physicians and the member support network to achieve positive outcomes.

Qualifications

• 2+ years in health care with care management or LTSS exposure
• Licensed clinical/social work credentials active in state of practice
• Valid driver license and reliable transportation for travel requirements
• Must have experience with individuals with disabilities, chronic conditions or substance use disorders

Responsibilities

• Complete member assessments within timelines, including home visits
• Facilitate waiver enrollment/disenrollment processes
• Develop and implement care plans with members, caregivers and providers
• Monitor care plan effectiveness and adjust as needed
• Promote integrated services and coordinate with ICT for approvals
• Assess medical necessity and authorize waiver services
• Evaluate benefits and funding sources
• Coordinate with interdisciplinary care team meetings
• Use motivational interviewing and Molina clinical guidelines
• Identify barriers to care and address psycho/social/financial obstacles
• Provide peer consultation and resources as needed
• Travel 25-40% locally as required
