Job summary

Trillium Health Resources is seeking a Care Manager – Innovations Waiver (IW) to join our Care Management Team. The role focuses on complex care coordination for members with Intellectual/Developmental Disabilities, potentially with co-occurring mental health or substance use needs, across children to adults.

The position requires NC licensure (QP or RN with MH/DD/SA experience or relevant degrees) and a willingness to travel within catchment.

Qualifications

• Fully licensed by the NC governing board regulating Human Services professions (QP status may apply).
• RN licensed in NC with MH/DD/SA experience acceptable.
• Masters or Bachelors in human services preferred; other degrees with MH/DD/SA experience acceptable.
• Driver’s license required.
• Minimum LTSS/HCBS coordination experience preferred.

Responsibilities

• Provide complex care coordination and monitoring to members with IDD with possible dual mental health/substance use needs.
• Assist members in understanding Medicaid/state-funded services and options.
• Collaborate with multidisciplinary team to coordinate services and ensure waiver requirements are met.
• Educate members and legally responsible individuals about services and rights.

Education

Master’s degree in human services

Bachelor’s degree in human services

RN license in NC

Licensed Human Services Professional (QP) in NC

Bachelor’s degree in a field other than human services with 4 years MH/DD/SA experience

Bachelor’s degree in any field with 2 years MH/DD/SA experience

Driver’s license
