Job summary

Alliance Health seeks a Care Manager I-TCL to deliver integrated care management for individuals with complex health needs in a primarily remote role, with onsite meetings in Morrisville, NC and travel across several counties.

You will coordinate across physical health, behavioral health, housing, and community resources, and document in JIVA to ensure person-centered plans are carried out and member outcomes improve.

Qualifications

• Bachelor’s degree and two years mh/dd/sa experience required.
• Master’s degree preferred with one year post graduate mh/dd/sa experience.
• Licensed in NC as LCSW/LCMHC/LMFT/LPA or RN with mh/dd/sa experience preferred.

Responsibilities

• Complete comprehensive assessments and planning for members.
• Develop Plans of Care and monitor progress.
• Coordinate with care teams and providers for service linkage.
• Document in JIVA and maintain timely updates.
• Support housing stability and tenancy through TCL processes.
• Travel as required to meet member and provider needs.
