Job summary

Alliance Health Plan in North Carolina is seeking a Care Manager I - TCL for a full-time remote role. The position focuses on coordinating care across physical, behavioral health, pharmacy, and social supports to help members stay in their preferred housing and reduce hospital visits.

The role may require travel within Cumberland, Johnston, Harnett, Wake and Durham counties and onsite reporting to the Morrisville office for meetings.

Qualifications

• Bachelor’s degree in Human Services and 2+ years mh/dd/sa experience
• Or Bachelor’s degree in a non-Human Services field with 4 years mh/dd/sa experience
• Or Master’s Degree in Human Services with 1 year mh/dd/sa experience
• Fully or Provisionally Licensed in North Carolina as LCSW/LCMHC/LPA/LMFT
• Licensed RN in NC with 4 years mh/dd/sa experience

Responsibilities

• Complete comprehensive assessments or Care Needs Screening at enrollment, yearly or at changes in condition
• Develop Plans of Care derived from the completed assessments
• Assign interventions/plans of care to team members
• Submit referrals to CCM for health/behavioral health needs
• Coordinate with team for transitions and housing supports
• Maintain records in JIVA and related systems
• Communicate with members to verify needs are met and update plans of care
• Travel within counties to support member needs
• Attend community and provider meetings as needed
