Job summary

Alliance Health is seeking a Care Manager I-TCL for a hybrid role serving Mecklenburg County, NC. You will coordinate integrated care across physical health, behavioral health, and housing supports, ensuring timely assessments, care planning, and referrals while traveling within Alliance communities as needed.

The role requires a bachelor’s degree in Human Services or equivalent MH/SA experience with licensure options or RN credentials; strong collaboration and documentation skills are

Qualifications

• Requires a Bachelor’s degree in Human Services or related field, with post‑bachelor mh/dd/sa experience if non‑Human Services.
• Preferred: Master’s in a Human Services field or licensure in NC (LCSW/LCMHC/LMFT) or RN with mh/dd/sa experience.
• Strong knowledge of motivational interviewing and community resources.

Responsibilities

• Conduct comprehensive assessments and care needs screenings at enrollment or changes in condition.
• Develop personalized care plans derived from assessments and coordinate across care teams.
• Coordinate with physical and behavioral health services to meet member needs and housing requirements.
• Refer to Integrated Health Consultant or care team for complex medical/behavioral needs.
• Document updates and plans in JIVA/Clive and ensure timely follow‑ups with members.
