Alliance Health in Charlotte, NC is hiring a Care Manager I – TCL to coordinate integrated care for individuals with special health care needs. The hybrid role requires onsite reporting and travel within Mecklenburg counties to support housing transitions and comprehensive care planning.

Minimum: a Bachelor’s degree in Human Services with 2+ years mh/dd/sa experience, or equivalent licensure; Master’s with related experience also acceptable. Licensure and certifications preferred.

Qualifications

• Bachelor’s degree in Human Services or related field plus 2 years mh/dd/sa experience.
• Or bachelor’s in non-human services with 4 years mh/dd/sa experience.
• Or master’s in human services with 1 year mh/dd/sa experience.
• Fully or Provisionally Licensed in NC as LCSW, LCMHC, LPA, or LMFT.
• Licensed RN in NC with 4 years mh/dd/sa experience.
• Preferred: NACCM, NADD-Specialist and/or CBIS Certification

Responsibilities

• Complete assessments and develop plans of care.
• Coordinate with care teams and service providers.
• Monitor, document, and ensure quality and compliance.
• Travel within Alliance communities as needed.
• Support tenancy stability and transitions.
• Attend community and stakeholder meetings.
• Maintain medical record compliance in required systems (JIVA, TCLD).
