Job summary

Alliance Health seeks a Care Manager I – non-Waiver to ensure integrated, whole-person care across physical health, behavioral health, and social needs. This remote, full-time role requires NC residency with occasional travel to the Morrisville office for on-site meetings or regional engagement.

The role emphasizes assessments, care planning, referrals, and collaboration with integrated health teams to improve outcomes and reduce hospital use.

Qualifications

• Bachelor’s degree or higher is required or equivalent experience in mh/dd/sa.
• Master’s degree is preferred for advanced practice.
• Licensed in North Carolina or RN with mh/dd/sa experience is acceptable.

Responsibilities

• Complete comprehensive assessments or Care Needs Screening at enrollment, yearly or at changes in condition.
• Develop Plans of Care derived from the completed assessments.
• Collaborate with the Care Management team to monitor plans and promote engagement.
• Submit referrals to Transition Support Team for medically complex needs.
• Coordinate with other teams to address Social Determinants of Health and payer issues.
• Maintain timely documentation and ensure compliance with state requirements.
