Job summary

Qualifications

• RN or SW license in Michigan (required)
• 3+ years in case/disease management
• 1+ year leadership experience in healthcare
• Experience coaching clinicians in fast-paced environments
• Proficiency in analyzing data trends
• Strong MS Office skills (Word, Excel, PowerPoint)

Responsibilities

• Oversee delivery of care to all members served by the location.
• Lead assessment and planning of member needs to achieve optimal wellness.
• Monitor staff performance against care coordination benchmarks.
• Identify members for case and disease management activities.
• Develop audit plans/tools to ensure compliance with state contracts.
• Recruit, hire, onboard, coach and mentor care coordination staff.
• Facilitate monthly area meetings and cross-department briefings.
