Job summary

The CVS Health Care Management Associate role supports comprehensive coordination of medical services, intake, screening and care plan implementation to promote efficient utilization of healthcare resources.

You will triage Care Team tasks, coordinate with nurses or medical directors and document all activities in accordance with risk, regulatory and accreditation standards. A strong emphasis on confidentiality and compliance is required.

Qualifications

• 2 years of experience in customer service, telemarketing or sales.
• Typing proficiency and ability to navigate multiple systems.
• Experience with Microsoft Word, Outlook and Excel.
• Ability to sit for long periods and perform computer-based tasks.

Responsibilities

• Triage care team tasks and perform initial review.
• Identify principal reason for admission and apply tools accordingly.
• Utilize Aetna systems to build and enter member information.
• Support development and implementation of Care Plans.
• Coordinate care delivery with nurses or medical directors in the most appropriate setting.
• Maintain thorough documentation meeting regulatory standards.
