Job summary

Harbor is seeking a Care Coordinator to provide comprehensive, multi-system care coordination for youth with complex needs and their families. The role is full-time (40 hours per week) and remote in the community, with a preference for residents of Huron County.

The ideal candidate has 3+ years of experience in community-based services or an advanced degree, strong communication and computer skills, and the ability to work across family systems, case management, and child development.

Qualifications

• 3+ years of experience in community-based services for children and youth or 2 years with an Associate or Bachelor degree; or 1 year with a Master degree.
• Experience in family systems, case management, counseling or therapy, child protection, or child development.
• Culturally competent with training to manage complex cases.
• Experience with youth experiencing SED, trauma, or co-occurring issues.
• Completion of high-fidelity wraparound training (COE) required within 90 days.
• Clear written and verbal communication; represent Harbor in health-care settings.
• Valid driver's license and insurable driving record; internet access requirements met.
• CPR/First Aid and NCI/CPI Certifications required within 90 days.

Responsibilities

• Engages youth and families for assigned caseload (1:10 ICC, 1:25 MCC).
• Links youth and family to community resources and supports.
• Conducts CANS assessments and develops care plans.
• Monitors and updates plans to address barriers.
• Facilitates Child and Family Team meetings with voice and choice.
• Coordinates access to clinical and psychiatric consultation.
• Develops safety-crisis plans with youth and family.
• Ensures engagement, contacts, and CME documentation are completed.
• Maintains flexible hours including evenings/weekends.
• Delivers services in home and community settings.
