Job summary

Mayo Clinic is seeking an experienced Registered Nurse in Minnesota to join a Primary Care team focused on chronic condition management and transitions from hospital to home. The RN will coordinate care across settings and support patient-centered health care delivery.

Qualifications

• RN licensed nurse with a BSN or equivalent accredited program.
• Current RN license in applicable state requirements.
• Ability to work flexible hours including days, evenings, nights, weekends and holidays.
• Experience with chronic conditions and care coordination preferred.
• Excellent communication and leadership capabilities.

Responsibilities

• Post-hospital follow-up calls for patients at risk of readmission.
• In-home visits for adult medical care coordination after discharge.
• Collaborate with primary care providers to implement care plans.
• Manage a defined patient panel with ongoing follow-up and monitoring.
• Educate patients on chronic conditions and medications for engagement.
