The role blends remote work with occasional field visits to support families in their communities. Ideal candidates will have a bachelor’s degree in a human service field and at least two years of direct service experience, with strong organizational and communication skills.

Qualifications

• Bachelor’s degree in human service fields or closely related field.
• Two years of direct service experience serving persons with DD/IDD or related disabilities.
• Two years’ healthcare/rehabilitation or related setting experience.
• Experience in completing assessments, service plan development and monitoring.

Responsibilities

• Provides information and assistance to link participants with medical, social and educational providers and services to address needs and goals.
• Conducts health, safety, social and environmental assessments per guidelines.
• Participates in work groups and committee activities as requested.
• Demonstrates effective organizational and communication skills.
• Willingness and ability to travel to serve families as required by state law.
