Job summary

Umpqua Health in Oregon seeks a Care Coordinator to provide comprehensive support for care management and coordination for Medicaid/Medicare members, including LTSS. You'll manage a caseload, perform in-home assessments, and collaborate with a multidisciplinary team to ensure high-quality, integrated care across the continuum.

The role requires strong organizational, communication, and compliance skills in a fast-paced setting.

Qualifications

• Active, unrestricted Oregon licensure as RN/LCSW or eligible health/human services degree for CCM
• 2+ years healthcare experience including LTSS and care management
• Valid driver’s license and reliable transportation
• Proficiency in Microsoft Office and ability to navigate EHR and care management systems
• CCM certification required within 18 months of hire

Responsibilities

• Perform comprehensive member assessments, including face-to-face and in-home visits as required
• Develop and implement individualized care plans in collaboration with members and providers
• Monitor care plans for effectiveness and adjust as needed
• Promote integration of services across behavioral health, LTSS, and community resources
• Evaluate benefits and advise on funding sources
• Facilitate interdisciplinary care team meetings and collaborate with team members
• Use motivational interviewing to educate and support members
• Identify barriers to care and address psychosocial, financial, and medical concerns
• Develop prevention plans to ensure member health and safety
• Maintain accurate documentation in EHR systems and ensure compliance
• Travel locally (25–40%) for member visits; mileage reimbursement provided
• Other duties as assigned
