Job summary

Qualifications

• Minimum 2 years healthcare experience with LTSS, and 1 year in care management or equivalent.
• Licensed or certified as required by state contract; active and unrestricted.
• In some states, a healthcare-related bachelor’s degree may be required.

Responsibilities

• Complete member assessments within timelines, including in-person visits.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care team.
• Monitor care plans and document interventions and outcomes.
• Promote integration of LTSS with behavioral health and community resources.
• Assess medical necessity and authorize waiver services.
