Job summary

Centene Corporation is seeking a Remote Care Coordinator for Long Term Care to support care management, member outreach, scheduling, and coordination with providers. The role includes occasional travel to Ohio offices for essential meetings and requires 2–4 years of related experience and a high school diploma or GED.

Pay ranges from $20.39 to $34.71 per hour; Centene offers comprehensive benefits including health insurance, a 401K/stock plans and tuition reimbursement.

Qualifications

• Requires a High School diploma or GED and 2–4 years in related experience.
• Preference for experience in managed care, physician offices or health systems.

Responsibilities

• Provide member outreach via phone or in home visits to discuss care plans and next steps.
• Coordinate care activities with healthcare providers, partners, and family/caregivers.
• Answer inquiries related to care plans and support continuity of care.
• Develop knowledge of care management services and respond to escalated issues.
• Document member records and share information with providers as required.
• Triage, adjust, and escalate complex requests with the care management team.
• Identify needs and refer to Care Manager or Disease Manager as appropriate.
• Educate members on benefits, resources, and services available.
