Job summary

Centene Management Company LLC offers a remote Long Term Care (LTC) Care Coordination role with occasional travel to Ohio offices for essential meetings. You will support member outreach, appointment scheduling, and coordination with providers while ensuring regulatory compliance.

Preference is given to candidates with managed care or healthcare operations experience and familiarity with medical terminology. Must reside in Ohio and be prepared for travel up to 10% to our Ohio offices.

Qualifications

• Requires a High School diploma or GED.
• Requires 2 - 4 years of related experience.

Responsibilities

• Coordinate LTC care through member outreach and scheduling.
• Perform telephonic outreach or home visits to engage members.
• Document care plans, coordinate with providers and caregivers.
• Maintain member records per state and regulatory requirements.
• Triage and escalate complex requests to care management.
