Job summary

SCAN Health Plan in Long Beach, CA is seeking a Care Coordinator to support aging in place by assessing needs, coordinating services, and guiding members through the health system.

You will perform assessments, referrals, and documentation while collaborating with providers and agencies. A high school diploma and 0–3 years in healthcare are required; Spanish preferred. Mostly remote work with some in-person coordination.

Qualifications

• High School Diploma required; some college preferred.
• 0-3 years in healthcare or customer service.
• Ability to apply motivational interviewing techniques.
• Strong written and verbal communication skills.
• Knowledge of CMS and DHCS regulations.
• Team collaboration and confidentiality awareness.

Responsibilities

• Perform assessments and screenings to determine interventions and referrals.
• Act as triage point with members, providers, and groups for benefits and access issues.
• Use motivational interviewing to understand goals and concerns.
• Coordinate care using benefits and community resources.
• Document assessments, care plans, and notes in multiple systems on time.
• Collaborate with county offices and agencies as needed.
• Demonstrate professionalism and empathy in all interactions.
• Support SCAN vision and team goals.
