Job summary

Esrun Health is seeking a Remote Care Coordinator to perform telephonic encounters with patients on behalf of our practice partners. This is a 1099 contractor position with flexible, set-your-own-hours and a part-time schedule, typically 20–30 hours per week, from home.

You will manage a patient panel, complete due diligence on all assigned patients and billable encounters, and help improve chronic disease management through proactive care.

Qualifications

• Compliance with HIPAA privacy laws.
• Ability to work independently as a contractor.
• Strong telephonic communication and patient-facing skills.

Responsibilities

• Perform telephonic encounters with patients for our practice partners.
• Manage a patient panel and maintain a minimum of 100 patients per calendar month within the first three months.
• Complete due diligence measures on all assigned patients and billable encounters.
• Contribute to improved revenue through care management and proactive patient engagement.
