Job summary

Alignment Health is seeking a compassionate, customer service oriented Remote Care Coordinator to join the outpatient case management team. You will work with the RN case manager and interdisciplinary team to support members with complex health needs, coordinate services, close care gaps, and ensure timely care. Schedule: Monday–Friday, 8:00 AM–5:00 PM Pacific Time.

Location: Fully Remote. Minimum 1 year healthcare experience required; bilingual Spanish preferred; HS diploma or GED required.

Qualifications

• Minimum 1 year experience in health care (health plan, medical office, IPA, MSSO).
• Able to read and interpret documents; write routine reports and correspondence.
• Strong communication, professional interaction, and teamwork.
• Bilingual English and Spanish preferred.
• Basic computer proficiency; typing at least 35 WPM; proficient with Microsoft Office (Outlook, Excel, Word).

Responsibilities

• Reach out to members by phone to assist with referrals, authorizations, HHC, DME needs, medication refills, and scheduling.
• Create cases and document tasks in the case management module for hospital/SNF discharges.
• Maintain confidentiality and coordinate with the nurse and care team to manage a member panel.
• Assist with outreach and updating member records, mailing or faxing correspondence to PCPs and specialists.
• Request and upload medical records from PCPs, specialists, hospitals, and other sources.
