Job summary

Alignment Health is seeking a Remote Care Coordinator for Transition of Care to support SNP members under the guidance of the RN Case Manager. You will contact members by phone to coordinate referrals, authorizations, home health, DME, medications, and appointments to ensure timely care.

The role requires bilingual English/Spanish, strong communication, and problem-solving skills within a fast-paced managed care environment.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree or equivalent work experience required.
• MBA preferred.
• Medical Assistant training or Medical Terminology training preferred.
• Bilingual English/Spanish required.

Responsibilities

• Reach out to members by phone to assist with referrals, authorizations, and scheduling.
• Create cases and complete assessments in Case Management for hospital and SNF discharges.
• Collaborate with nurse and Case Manager to coordinate care for SNP members.
• Manage alerts and update Case Manager on changes in condition, admission, or discharge.
• Maintain and update member records and communicate with PCPs and specialists.
• Attend Interdisciplinary Team Meetings and participate in member presentations.
