Alignment Health is seeking a Care Coordinator to support RN Case Manager in coordinating care for SNP members. You will reach out to members, assist with referrals and authorizations, and maintain comprehensive records to ensure high-quality care.

The role involves collaboration with an interdisciplinary team, managing cases from discharge to follow-up, and helping close care gaps while maintaining confidentiality and patient advocacy.

Qualifications

• High school diploma or GED required; Bachelor’s degree preferred or four years of additional experience.
• Bilingual English/Spanish; strong communication and interpersonal skills.
• Knowledge of managed care plans and Medi-Cal beneficial.

Responsabilités

• Assist with referrals, authorizations, and care coordination tasks.
• Create cases and track discharges in Case Management system.
• Collaborate with RN Case Manager and interdisciplinary team.
• Maintain member records and coordinate with PCPs and specialists.
• Engage with SNP members and manage care gap closures.
• Meet deadlines and maintain confidentiality.
