Job summary

The Columbus Organization, LLC is seeking a Team Leader for a remote care coordination role serving Georgia residents within Bartow, Catoosa, Chattooga, Cherokee, Cobb, Douglas, Floyd, Fulton, Gordon, Haralson, Laurens, Paulding, Pickens, Polk, and Walker counties.

You will guide a team, oversee programs, ensure timely state contract deliverables, supervise staff, and collaborate with provider agencies to coordinate services effectively.

Qualifications

• Bachelor’s degree in a human services field (or four years of relevant exp).
• Minimum of two years of care coordination experience.
• Valid state driver’s license and access to a personal vehicle.
• Must reside in the assigned territory; travel required.
• Evening/weekend work may be required.

Responsibilities

• Lead and supervise assigned staff; conduct monthly supervision and annual evaluations.
• Oversee program delivery, ensure timely state contract deliverables, and maintain regulatory compliance.
• Coordinate with provider agencies to ensure timely services and quality outcomes.
• Participate in budget planning, reimbursements, and travel arrangements.
• Represent the organization in meetings and community engagements.
