Azalea Homecare is seeking a detailed and organized Care Coordinator in Lawrenceville, GA to manage client services, caregiver scheduling, and day-to-day communications. This role acts as the primary liaison for clients, families, and caregivers and collaborates with operations, nursing, recruiting, onboarding, and billing.

The ideal candidate communicates well, stays organized, and can handle multiple priorities in a fast-paced home care environment.

Qualifications

• Experience in home care, healthcare scheduling, staffing, client services or related role preferred.
• Strong communication and customer-service skills.
• Excellent organization, problem-solving and follow-through.

Responsibilities

• Serve as primary contact for clients, families and caregivers; manage communications.
• Create and maintain caregiver schedules based on client needs and availability.
• Complete weekly client check-ins by phone, email or in person.
• Follow concerns through resolution and escalate significant service or staffing issues.
• Coordinate client-to-caregiver introductions and onboarding support.
• Assist with onboarding, nursing coordination, scheduling, and billing-related tasks.
• Maintain accurate client information and notes in the scheduling system.
