The Columbus Organization in Indiana seeks a Team Leader to guide and manage a care coordination team, providing strategic direction and administrative oversight for coordinated service delivery. You will supervise staff, ensure contract deliverables, and partner with providers and state agencies to improve outcomes.

This mobile, field-based role requires regular travel within the assigned counties and occasional evenings or weekends, with remote administrative work when not visiting clients.

Competenze

• Bachelor’s degree in a human services field or equivalent experience.
• Minimum two years of care coordination experience for individuals with developmental disabilities.
• Valid state driver’s license and access to a personal vehicle.
• Must reside in the assigned territory and be mobile for travel.
• Occasional evening and weekend work may be required.
• This role may involve collaboration with multiple providers and state agencies.

• Promote efficient use of staff resources to support billable services and customer satisfaction.
• Contribute to planning and strategy for growth and improvement.
• Support efforts to secure new funding and expand contracts.
• Strengthen partnerships with providers, agencies, and community organizations.
• Represent the organization at conferences and stakeholder meetings.
• Ensure staff adherence to standards set by accrediting bodies (e.g., CARF).
• Support quality assurance initiatives and continuous improvement efforts.
