Humana Inc. (OneHome) seeks a Care Management Support Assistant to administer care management, providing non-clinical support for assessments and coordinating with members and families to access appropriate resources.

The role involves reviewing referrals, verifying benefits, and supporting care with providers. This remote, full-time position requires HIPAA awareness and collaboration with Pre-Authorization, Utilization Management, Billing, Pharmacy and Home Care to ensure timely services and

Qualifications

• 1–3 years of Home Health or Infusion Pharmacy experience required.
• Wound care/Ostomy supplies experience preferred.
• Microsoft Office (Word, Outlook, Excel) experience preferred.
• Knowledge of Medicare/Medicaid & commercial insurance required.
• Experienced with medical authorizations required.
• Knowledge of medical billing preferred.
• Excellent customer service and communication skills.
• Passionate about contributing to an organization focused on continuously improving consumer experiences.

Responsibilities

• Assist with administration of care management and non-clinical support for assessments and evaluations.
• Review inbound referrals and correspondence for processing and resolution.
• Verify benefits coverage and eligibility for services per Health Plan contract.
• Coordinate timely delivery of care with providers and communicate with patients.
• Maintain HIPAA and confidentiality; document interactions with providers, members, and referrals.
• Collaborate with Pre-Authorization, Utilization Management, Billing, Pharmacy and Home Care.
