CVS Health is seeking a Case Management Coordinator in Ohio to support members through care coordination and education, with travel within the region up to 50%.

The role emphasizes collaboration with case managers, supervisors, medical directors and community teams to overcome barriers and improve health outcomes for children and families served by OhioRISE.

40-hour workweek with standard hours and flexibility to meet member needs; field-based telework within Ohio region.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint).
• Willing to travel up to 50% within Northeast/Northwest Ohio; reliable transport.

Responsabilités

• Coordinate and implement care plan activities and monitor progress.
• Evaluate member needs and eligibility using care management tools.
• Collaborate with case managers, supervisors, Medical Directors and other programs to overcome barriers.
• Identify and escalate quality of care issues.
• Provide coaching and support to empower informed health decisions.
