Full Time Care Coordination Remote, GA, US

Salary: $58,656.00 Annually

This position is a hybrid position in Atlanta Georgia.

Summary of position

The Team Leader is responsible for guiding and managing the operations of their assigned team. This includes providing strategic direction, administrative oversight, and ensuring the effective coordination of service delivery for team members under their supervision. They are accountable for the overall performance of the team, including staff management, program implementation, and operational efficiency.

Key responsibilities include planning, assigning, evaluating, and reviewing staff work, as well as contributing to service coordination and continuous improvement efforts. The Team Leader collaborates with provider agencies to ensure services are delivered in a timely and effective manner.

Additionally, the Team Leader may represent The Columbus Organization on committees, task forces, and in public forums. They engage with external stakeholders—including individuals and families, community representatives, state offices, and service providers—by providing information, addressing inquiries, resolving complaints, and explaining policies and programs.

Essential Functions
  • Leadership & Supervision
  • Supervise, mentor, and evaluate assigned staff.
  • Conduct monthly supervision and annual performance reviews.
  • Support staff in goal setting and professional development.
  • Assist in recruitment and hiring processes.
  • Program Oversight
  • Ensure timely completion of state contract deliverables.
  • Monitor staff compliance with company policies and regulatory requirements.
  • Conduct team meetings and site visits to support service delivery.
  • Provide coverage during staff transitions or absences.
  • Administrative Responsibilities
  • Oversee workflow, assign tasks, and assess staff performance.
  • Ensure accuracy and timeliness of mandatory reporting.
  • Participate in budget planning and resource allocation.
  • Review and verify reimbursements and travel requests.
  • Collaborate with leadership on program evaluations and quality assurance.
  • Strategic Planning & Development
  • Develop and implement annual work plans aligned with organizational goals.
  • Partner with leadership to advance strategic initiatives and set priorities.
  • Analyze team performance and implement corrective actions as needed.
  • Promote efficient use of staff resources to support billable services and customer satisfaction.
  • Business Development & Community Engagement
  • Contribute to planning and strategy for growth and improvement.
  • Support efforts to secure new funding and expand contracts.
  • Strengthen partnerships with providers, agencies, and community organizations.
  • Represent the organization at conferences and stakeholder meetings.
  • Compliance & Quality Assurance
  • Ensure staff adherence to standards set by accrediting bodies (e.g., CARF).
  • Support quality assurance initiatives and continuous improvement efforts.
Position Requirements
  • Bachelor’s degree in a human services field (such as social work, psychology, special education, nursing, or a related discipline), or four years of relevant experience in a similar role (e.g., case management, behavioral health support, community outreach, counseling services, residential care, or client advocacy).
  • Minimum of two years of experience in care coordination for individuals with developmental disabilities
  • Valid state driver’s license, automobile insurance, and access to a personal vehicle.
  • Must reside in the assigned territory (exceptions may be considered).
  • Mobile position requiring regular travel and exposure to varying weather conditions.
  • Occasional evening and weekend work may be required.
  • Georgia:
  • Bachelor’s degree in a human services field (e.g., social work, psychology, special education, nursing, or related).
  • Minimum of one year of experience in case management or coordination services for individuals with developmental or related disabilities.
  • A master’s degree in a related field may substitute for the required experience.
  • Indiana:
  • Bachelor’s degree in human services preferred.
  • Minimum of two years of experience in care coordination for individuals with developmental disabilities.
  • Experience in large-scale case management oversight is strongly preferred.
