Job summary

Mount Sinai Health System seeks a Services Management Coordinator to support care coordination and patient navigation across assigned programs. The role provides administrative support to providers, nurses, medical directors, and case management teams to ensure seamless patient experiences within the program.

The coordinator educates patients on program benefits, assists with prior authorizations, and helps manage documentation in the EMR and program benefits systems to plan next steps in care.

Qualifications

• Bachelor’s degree or equivalent experience.
• 3 years’ experience in healthcare or customer service.
• Experience coordinating care or navigation is preferred.

Responsibilities

• Plays a lead role in care navigation and explains program benefits to patients.
• Sets goals with patient and care team to coordinate care and tracks follow-up actions.
• Ensures provider orders are authorized and align with covered patient services.
• Educates patients on pharmacy benefits and assists with prior authorizations when needed.
• Organizes correspondence and future program plans in the EMR and benefits systems.
• Facilitates patient navigation between program visits and other encounters.
• Educates patients to ensure understanding of program benefits and coverage.
• Suggests process improvements and participates in pilot programs to enhance patient engagement.
