Qualifications

• Bilingual is desirable.
• Experience with Medicare patients.
• Comfortable using iPads for charting and documentation.
• Strong verbal and written communication skills.
• Valid driver’s license with clean record.

Responsabilités

• Perform routine primary care nursing tasks: vital signs, point of care tests, charting, and maintaining records.
• Educate patients using motivational communication to drive healthy behaviors.
• Provide preventive health and disease management coaching and guidance.
• Deliver care during home visits and ensure a positive patient experience.
